Background: Plasma phosphorylated tau-217 (pTau-217) measurements for use in Alzheimer disease (AD) identification require thresholds to define positivity and there exist different approaches to operationally defining the boundary. We compared amyloid {beta} (AB) PET-anchored and distribution-based positivity cut-off values and explored how these mapped onto latent biomarker states. Methods: We analysed plasma pTau-217 measured in the Bio-Hermes-001 cohort (N = 990) using an immunoassay (Lilly) and mass spectrometry assay (University of Gothenburg).
